Take a moment and read the docs; the sample config is exactly that. Take a
walkthrough of our HTML documentation in the 1.99 release, and read the man
page for ipsec.conf.
For a host-to-host connection, you wouldn't specify anything using the
"leftsubnet" or "rightsubnet" pararmeters; you're not trying to make an
encrypted tunnel on behalf of other machines.

No particular reason. The netmask can vary from /0 to a /32 as needed.
